Methodist Healthcare Ministries of South Texas: Juntos for Better Health

Juntos for Better Health (Juntos) is a partnership of four community service providers that developed a coordinated health care delivery system among multiple partners in the Texas city of Laredo as well as in the Texas counties, Webb, Zapata, and Jim Hogg. The coordinated system addresses the lack of centralized and comprehensive services in the region.

TRIP for Salud y Vida

The TRIP for Salud y Vida program was implemented and led by the Rural Economic Assistance League (REAL) and its partners. It is an expansion of the Project Salud y Vida program. The program is designed to improve health outcomes, specifically blood pressure results, through enhanced integrated services.

Salud y Vida 2.0

In September 2016, the University of Texas Health Science Center at Houston (UT Health SPH) and partners implemented an enhanced version of the Salud y Vida chronic care management program. The enhanced version integrated primary and behavior healthcare with home and community-based wraparound services.

Final Evaluation Report: Nuestra Clinica del Valle

Nuestra Clínica del Valle (NCDV) implemented the NuCare program, which fully integrates behavioral health (IBH) and physical health at four of its clinics in the Rio Grande Valley. The program is intended to improve the health status of patients with obesity, diabetes, and/or depression. This study specifically looks at the program at the Mission clinic site in Mission, TX.

Mercy Ministries of Laredo Sí Three: Integration of 3-D Health Services Final Report

Mercy Ministries of Laredo (Mercy) is a primary healthcare clinic located in Webb County, Texas. The clinic provides healthcare and health education to some of the poorest neighborhoods and unincorporated areas in the United States. Mercy began implementing the Sí Three program and the evaluation study in the Laredo clinic in January 2016. The Sí Three program expanded Mercy’s efforts to integrate behavioral health, including optional faith-based behavioral health services, into primary healthcare services.
